What is IGFBP-3? Understanding the Growth Hormone Partner

In my clinical practice, when a child's growth curve flattens or an adult develops acromegalic features, I often turn to IGFBP-3. Insulin-like growth factor binding protein 3 (IGFBP-3) is the main carrier protein for IGF-1 in the bloodstream. It acts like a taxi, binding over 90% of circulating IGF-1, controlling its half-life and delivery to tissues.

Unlike growth hormone (GH), which is released in pulses and is hard to catch, IGFBP-3 levels remain relatively stable throughout the day. This makes it a reliable surrogate for growth hormone status. The test is formally known by LOINC code 21147-5.

Screening for Growth Hormone Deficiency (GHD)

When a child is growing slowly—below the 3rd percentile or decelerating—a low IGFBP-3 suggests the pituitary isn't making enough GH. My patients often ask, 'Could it just be a late bloomer?' But if both IGF-1 and IGFBP-3 are low, we proceed to GH stimulation testing.

Monitoring Acromegaly and GH Excess

In adults, persistently high IGFBP-3 levels, along with high IGF-1, point toward acromegaly—a condition caused by a pituitary tumour overproducing GH. After surgery or medication, we track IGFBP-3 to see if treatment is working.

Nutritional Assessment

Severe malnutrition, liver disease, or catabolic states suppress IGFBP-3 because the liver needs adequate nutrition to produce it. I sometimes use it as a marker of overall nutritional status in hospitalised patients.

Interpreting Abnormal IGFBP-3 Levels

Low IGFBP-3 – What It Means

A low result can indicate growth hormone deficiency, hypopituitarism, or poor nutrition. In children, I always check height velocity; in adults, I consider conditions like liver cirrhosis or chronic renal failure. Very low levels in a child with normal height may simply be a benign 'constitutional delay'.

High IGFBP-3 – Causes and Concerns

Elevated IGFBP-3 is most often seen with acromegaly or gigantism. However, pregnancy naturally raises levels, and some insulin-resistant states (like diabetes) can also cause a mild increase. If IGF-1 is also high, we do an oral glucose tolerance test for GH suppression.

IGFBP-3 and Cancer Risk: Is There a Link?

This is a question I hear frequently. Some epidemiological studies suggest that higher IGFBP-3 levels may be associated with a reduced risk of breast and prostate cancer, possibly because it binds IGF-1 and limits its growth-promoting effect. However, the data is not yet strong enough to use IGFBP-3 as a cancer screening test. More research is needed.

How to Prepare for the IGFBP-3 Test

No special preparation is needed. The test is a simple blood draw. Fasting is not required, though if your doctor is also checking glucose or lipids, you may need to fast. Let your doctor know about any medications, especially estrogen therapy or glucocorticoids, as they can alter results.

Limitations of the IGFBP-3 Test

No test is perfect. IGFBP-3 can be normal in some children with mild GH deficiency, and it can be low in obese children even when GH is normal. That's why I never use it alone; I combine it with IGF-1 and clinical findings. For the most accurate assessment, a GH stimulation test is still the gold standard.
