Explicit Consent Declaration for Processing Special Category Personal Data

IMPORTANT WARNING:

This document has been prepared to obtain your explicit consent for the processing of your special category personal data (health data). Please read carefully.

1. Scope and Definition

This explicit consent form has been prepared to obtain your consent for the processing of your special category personal data by the e-Tahlil+ platform, within the scope of the Personal Data Protection Law No. 6698 ("KVKK").

Special Category Health Data:

Laboratory test results, analysis values, and health analyses based on them are classified as special category personal data.

2. Health Data to Be Processed

The following special category health data will be processed within the scope of your platform use:

a) Laboratory Results Complete blood count, biochemistry, hormones, vitamins, minerals, and other laboratory tests

b) Biochemical Parameters Blood sugar, cholesterol, liver enzymes, kidney function tests, and other biochemical values

c) Physical Measurements Height, weight, body mass index, and related health parameters

d) Demographic Health Data Date of birth, gender, and information used for age-based reference ranges

e) Analysis Results Correlation analyses, trend reports, and health assessments generated by the platform

3. Processing Purposes

Your health data will be processed exclusively for the following purposes:

  1. Analysis and interpretation of your laboratory results
  2. Performing inter-parameter correlation analyses
  3. Tracking your health trends over time
  4. Comparison with reference ranges based on age and gender
  5. Providing AI-supported health consulting
  6. Creating personalized health reports
  7. Proactive risk notification and early warning

IMPORTANT NOTE:

Your health data will not be used for marketing, advertising, or profiling purposes; it will only be processed for the provision of health analysis services.

4. Data Transfers

Your health data may be transferred to the following recipients for the specified purposes:

a) Cloud Storage Providers

Your data is stored on secure cloud servers protected with AES-256 encryption.

b) Algorithmic Analysis Services

Anonymized or pseudonymized versions of data are transferred to the algorithmic evaluation system for analysis.

c) Legal Obligations

Transfers may be made to authorized public institutions in cases of legal obligation.

5. Withdrawal of Consent

Pursuant to Article 7 of KVKK, you have the right to withdraw your explicit consent at any time. In the event of withdrawal:

  • Processing of your health data will be stopped
  • Platform health analysis services will become unavailable
  • Your account will be deactivated
  • Your existing data will be deleted after legal retention periods expire

6. Security Measures

The following security measures are implemented to protect your health data:

AES-256 Encryption

All health data is stored encrypted with AES-256 standard

TLS 1.3

All data transmission is encrypted with TLS 1.3 protocol

JWT Authentication

Secure token-based authentication system is used

Row-Level Security

Each user can only access their own data

7. Explicit Consent Declaration

I have read and understood this document and give my explicit consent for the processing of my special category personal data (health data) for the purposes stated above.

I acknowledge that my consent is given voluntarily and that I can withdraw it at any time.
